Diarrhea during menstruation is relatively common among women and is medically termed “menstruation-associated diarrhea.” From a Western medical perspective, this condition is attributed to hormonal imbalances occurring during the menstrual cycle. In Traditional Chinese Medicine (TCM), menstruation-associated diarrhea is often linked to constitutional deficiencies of the Spleen and Kidney. Spleen Qi deficiency impairs the Spleen’s function of transforming and transporting water-dampness, leading to diarrhea; Kidney deficiency results in a weakened Mingmen (Life Gate) fire, failing to warm and support the Spleen Yang—also contributing to diarrhea. For severe cases, Gu Chang Zhi Xie Wan (Solidify the Intestines and Arrest Diarrhea Pills) may be recommended. This formula harmonizes the Liver and Spleen while astringing the intestines and relieving pain. It is indicated for conditions such as Liver-Spleen disharmony, diarrhea with abdominal pain, and chronic nonspecific ulcerative colitis presenting with the aforementioned symptoms.

During menstruation, attention must be paid to menstrual hygiene. Sanitary products should be changed regularly; the external genital area must be kept clean and dry; undergarments should be changed frequently to prevent prolonged exposure of the vulva to menstrual blood, which could facilitate bacterial retrograde infection and lead to pelvic inflammatory disease. Sexual intercourse should be avoided during menstruation, as the cervical os is open, increasing susceptibility to pathogenic bacterial retrograde infection and raising the risk of endometritis and pelvic inflammatory disease. Light physical activity is permissible and even beneficial during menstruation, as it helps promote menstrual flow. Diet should remain light and bland; cold, raw, spicy, or otherwise irritating foods should be minimized.
