Some patients with hyperthyroidism experience tinnitus, possibly due to excessive sympathetic nervous system activation. Hyperthyroidism is a metabolic disorder that accelerates the body’s metabolism, impairs neural function, and disrupts normal blood circulation—leading to tissue hypoxia and ischemia, which may trigger tinnitus. Typically, this tinnitus gradually improves as hyperthyroidism is effectively treated.

It is widely known that the causes of hyperthyroidism may include genetic predisposition, excessive iodine intake, excessive psychological stress, compromised immune function, and insufficient sleep. But can hyperthyroidism cause tinnitus?

Can hyperthyroidism cause tinnitus?

Some patients with hyperthyroidism experience tinnitus, possibly due to overactivation of the sympathetic nervous system. Hyperthyroidism is a metabolic disorder that accelerates overall metabolism, exerts inhibitory effects on the nervous system, and disrupts normal blood circulation—leading to tissue hypoxia and ischemia, which may manifest as tinnitus. Typically, such tinnitus gradually improves as hyperthyroidism is effectively treated. During active hyperthyroidism, if other potential causes—such as vascular factors—are ruled out, tinnitus may arise from occlusion of arteries near the ear or face.

Hearing impairment can also trigger tinnitus; the most common cause is presbycusis (age-related hearing loss), which typically begins around age 60 and progresses gradually, often accompanied by tinnitus. Middle ear abnormalities—including eustachian tube dysfunction—may contribute to tinnitus. Additionally, certain head and neck traumas or temporomandibular joint disorders may also precipitate tinnitus.

Mild cases are typically managed with antithyroid medications, such as methimazole or propylthiouracil, which effectively control disease progression. For patients with severe, recurrent disease, surgical thyroidectomy may be indicated—not only to control hyperthyroidism but also to prevent complications such as goiter enlargement or malignant transformation.
