Introduction
If you have never received the hepatitis B vaccine before, and all five hepatitis B tests are negative—including a negative hepatitis B surface antibody—this is considered normal. However, if you were previously vaccinated but your hepatitis B surface antibody has turned negative, it means the vaccine is no longer effective and you have lost your immunity against hepatitis B. In such cases, you must receive a vaccine injection at the immunization station based on the results of the five-item hepatitis B test panel.

Some individuals undergo hepatitis B–related testing in daily life. Upon receiving their test results, they notice that the hepatitis B surface antigen (HBsAg) is reported as negative but are uncertain whether this result falls within the normal range. So, is a negative HBsAg result normal?

Is a negative hepatitis B surface antigen result normal?

If an individual has never received the hepatitis B vaccine and all five hepatitis B serological markers (the “hepatitis B panel”) are negative—including HBsAg, hepatitis B surface antibody (anti-HBs), hepatitis B e-antigen (HBeAg), hepatitis B e-antibody (anti-HBe), and hepatitis B core antibody (anti-HBc)—then a negative HBsAg result is considered normal. However, if the person previously received the hepatitis B vaccine but now tests negative for both HBsAg and anti-HBs, this suggests that the prior vaccination no longer confers protective immunity, meaning the individual has lost immunity against the hepatitis B virus.

In such cases, it is essential to visit a local disease prevention and control center (or designated vaccination clinic) to receive a booster dose of the hepatitis B vaccine. Vaccination is only recommended when all five hepatitis B markers are negative. If any of the following are positive—HBsAg, anti-HBc, or anti-HBe—this indicates either past or current hepatitis B virus infection, and hepatitis B vaccination is not advised under these circumstances. Patients are encouraged to maintain a positive and optimistic mindset, avoid excessive mental stress, engage in appropriate physical exercise to strengthen overall health, and adhere to regular, healthy sleep habits.
