Site Selection and Localization Methods for Intramuscular Injection in Adults
The common sites for intramuscular injection include the gluteus maximus, gluteus medius and gluteus minimus, the lateral thigh muscle, and the deltoid muscle. Localization methods include the cross method, triangular area, lateral mid-thigh region, and outer aspect of the upper arm.
1. Gluteus Maximus
The cross method is primarily used: starting from the gluteal cleft, draw a horizontal line across it, and a vertical line from the highest point of the iliac crest. The injection site is located in the upper outer quadrant formed by these lines.
2. Gluteus Medius and Gluteus Minimus
Place the index finger on the anterior superior iliac spine and the middle finger along the lower border of the iliac crest. The triangular area formed between the iliac crest, index finger, and middle finger serves as the injection site.
3. Lateral Thigh Muscles
On the outer side of the thigh, the injection site for adults is typically located 10 cm below the buttocks to 10 cm above the knee.
4. Deltoid Muscle
On the outer aspect of the upper arm, approximately 2–3 finger widths below the acromion process. However, this site is generally used only for low-volume injections.
Patients are advised to maintain cleanliness at the injection site after an intramuscular injection and avoid prolonged exposure to water to prevent infection.
