Introduction
After having an IUD for twenty years, if a woman's menstrual flow remains normal without significant discomfort such as lower back pain or increased menstrual bleeding, and the IUD is still within its effective period with no plans for pregnancy, removal may not be necessary for now. However, if the IUD has reached the end of its recommended duration, or if there are plans for pregnancy, abnormal symptoms such as increased menstrual flow, lower back pain, abdominal pain, or if menopause has already occurred, it is important to go to the hospital promptly for IUD removal.

Whether or not to remove an intrauterine device (IUD) after 20 years should be decided based on the IUD's expiration date, the woman's physical condition, and whether she has further fertility plans. It is recommended to undergo an examination at a正规 hospital before making a decision. Specific analysis is as follows:

1. Not necessary

If after 20 years of IUD use, the woman’s menstrual flow remains normal without obvious discomfort such as lower back pain or increased menstrual bleeding, and if the IUD is still within its valid period and she has no plans for future pregnancy, removal is temporarily unnecessary and the IUD can continue to be used for contraception.

2. Necessary

If the IUD has reached its expiration date, or if the woman wishes to become pregnant, experiences abnormal symptoms such as increased menstrual flow, lower back pain, abdominal pain, or has already entered menopause, the IUD should be promptly removed at a hospital to avoid potential risks to her health.

After insertion of an IUD, regular B-ultrasound examinations are recommended to monitor its position. If any abnormalities occur, the IUD should be removed at the hospital during the period 3–7 days after menstruation ends, avoiding sexual intercourse prior to the procedure.
