Whether a subcapsular hepatic hematoma can resolve spontaneously depends on the specific circumstances. If the injury is mild, the hematoma is small in size, and there are no other symptoms, it usually gets absorbed gradually by the surrounding tissues and resolves on its own. However, if the abdominal trauma is severe, resulting in a large liver hematoma that causes an upper abdominal mass, pain in the liver area, or similar symptoms, it typically will not resolve spontaneously.

2. Cannot resolve

In such cases, the hematoma generally does not resolve spontaneously and may even lead to chronic progressive anemia. Prompt medical evaluation is recommended to prevent internal bleeding.
