Introduction
Menstruation occurs due to the shedding of the endometrial lining. During menstruation, large pieces of the shed endometrium are expelled along with menstrual blood, which may result in the sensation of something falling out. After a miscarriage, pregnancy-related tissue and clotted blood expelled from the uterine cavity may be mistaken for normal menstrual bleeding. Endometrial polyps can also shed along with the detached endometrial lining and be expelled with menstrual flow, appearing as blood clots or meat-like tissue.

It is possible to experience something falling out during menstruation, which could be a normal physiological phenomenon or related to pathological conditions such as miscarriage with expulsion of pregnancy tissue or endometrial polyps. An ultrasound examination is needed to determine the cause and guide appropriate management. Specific analysis is as follows:

I. Physiological Phenomenon

Menstruation occurs due to shedding of the endometrial lining. During menstruation, if large pieces of endometrial tissue are shed and expelled along with menstrual blood, it may feel like something is falling out.

II. Pathological Phenomena

1. Expulsion of Pregnancy Tissue After Miscarriage
After a miscarriage, pregnancy-related tissues and clotted blood within the uterine cavity may be expelled and mistaken for regular menstrual flow. Color Doppler ultrasound is recommended to evaluate the condition. Under medical guidance, medications such as Yimucao Granules or Xinshenghua Granules may be used to promote the expulsion of residual tissue.

2. Endometrial Polyps
Endometrial polyps may detach along with the shedding endometrium and be expelled with menstrual blood, appearing as blood clots or fleshy tissue. Hysteroscopic polypectomy at a hospital is generally recommended.

In addition, this could also be caused by other conditions such as uterine fibroids. If tissue-like material is passed during menstruation, close monitoring is necessary. If accompanied by abdominal pain or increased bleeding, prompt medical evaluation and treatment are required.
