Introduction
Poor R-wave progression typically indicates ventricular hypertrophy, which may result from hypertension causing excessive cardiac load. Myocardial damage or infection can lead to myocardial thickening, resulting in poor R-wave progression. Inadequate coronary blood supply leading to myocardial ischemia or necrosis can also cause myocardial thickening and subsequent poor R-wave progression. In addition, aortic stenosis and aortic regurgitation may also contribute to this condition.

Generally, poor R-wave progression usually indicates ventricular hypertrophy, which may be caused by conditions such as hypertension, myocarditis, or coronary heart disease. Specific analyses are as follows:

1. Hypertension

Long-term hypertension can overload the heart, leading to ventricular hypertrophy.

2. Myocarditis

Myocardial damage or infection can result in myocardial thickening, thereby causing poor R-wave progression.

3. Coronary Heart Disease

Inadequate blood supply from coronary artery disease leads to myocardial ischemia and necrosis, which can also cause myocardial thickening and result in poor R-wave progression.
