Introduction
1. Appendicitis: In the early stage of onset, pain initially appears in the right upper abdomen and gradually shifts to the right lower quadrant. 2. Enteritis: Symptoms include abdominal pain and diarrhea, with abdominal discomfort often localized to the right lower quadrant. 3. Hepatobiliary disorders: The liver and gallbladder are anatomically located in the right upper quadrant (not the right lower quadrant); however, pathology involving these organs may refer pain to the right lower quadrant. 4. Gynecological conditions, such as oophoritis or endometriosis.

Many people experience pain in the lower right abdomen in daily life. However, this pain is often mild and tolerable, leading most individuals to simply endure it. Since numerous conditions can cause right-sided abdominal pain, identifying the underlying cause is essential to guide targeted, symptom-specific treatment. So, what could be causing a dull, persistent ache in the right abdomen? Below, we provide answers.

What Causes a Dull, Persistent Ache in the Right Abdomen?

1. Appendicitis

Appendicitis is a serious gastrointestinal condition. In its early stage, pain typically begins in the upper right abdomen and gradually shifts to the lower right quadrant. Initially, the pain may be mild but is often accompanied by nausea, vomiting, and generalized fatigue. Without prompt treatment, appendicitis can progress to appendiceal perforation—a potentially life-threatening complication. Therefore, if lower right abdominal pain is accompanied by symptoms such as nausea or vomiting, appendicitis should be strongly suspected.

2. Enteritis

Poor dietary habits—such as consuming unclean food or water, or eating raw or cold foods—can trigger enteritis. Typical symptoms include abdominal pain and diarrhea, with discomfort often localized to the lower right abdomen. If left untreated, enteritis may lead to dehydration and pose a threat to overall health. Thus, lower right abdominal pain accompanied by diarrhea warrants suspicion of enteritis.

3. Hepatobiliary Disorders

The liver and gallbladder are located in the right upper quadrant—not the lower right abdomen—but referred pain from hepatobiliary disease may manifest in the lower right abdomen. Because the liver has relatively few pain-sensitive nerve endings, pain originating from this organ often indicates significant damage. Prompt medical evaluation is therefore crucial. Hence, lower right abdominal pain must not be ignored or tolerated, especially when hepatobiliary disease is suspected, to prevent worsening of the condition.

4. Gynecological Conditions

Gynecological disorders are common among women and frequently present with lower right abdominal pain. However, because some conditions—such as oophoritis or endometriosis—cause only mild discomfort, they are easily overlooked. These disorders may also be associated with abnormal vaginal discharge. Therefore, women experiencing both abnormal vaginal discharge and lower right abdominal pain should suspect gynecological pathology and seek timely medical evaluation.
