What causes chest tightness and shortness of breath during sleep at night?
A relatively common symptom is experiencing chest tightness, shortness of breath, or a sensation of being unable to breathe during sleep at night. This may stem from several underlying causes—primarily respiratory or cardiovascular disorders.
One common cause is asthma. Asthma symptoms often worsen at night, leading to nocturnal chest tightness, shortness of breath, and wheezing.
Another frequent cause is obstructive sleep apnea–hypopnea syndrome (OSAHS). Many people are familiar with snoring; however, snoring often reflects nocturnal episodes of breathing cessation or upper airway obstruction—manifested as loud snoring—ultimately resulting in hypoxia. Consequently, individuals may awaken with a sensation of breathlessness. This condition is particularly prevalent among individuals with obesity.
Cardiovascular conditions—including coronary artery disease and heart failure—may also cause nocturnal dyspnea. In such cases, the underlying issue is typically left-sided heart failure. Therefore, accurate diagnosis of the specific etiology is essential to guide appropriate, targeted management.
