Midnight gastric pain is clinically referred to as “nocturnal acid breakthrough.” During the night, the vagus nerve becomes more active, leading to increased gastric acid secretion. In patients with gastrointestinal disorders—such as gastritis, gastric ulcers, or duodenal bulb ulcers—this excess gastric acid can irritate and damage the mucosa, stimulating the autonomic nervous system and thereby causing abdominal pain.

Nocturnal gastric pain—so severe that it awakens patients from sleep—is actually very common in clinical practice. This phenomenon is clinically termed “nocturnal acid breakthrough,” referring to increased gastric acid secretion during the night, when the vagus nerve is relatively more active.

In patients with gastrointestinal disorders—such as gastritis, gastric ulcers, or duodenal bulb ulcers—excess gastric acid can irritate and damage the mucosa, stimulating the autonomic nervous system and thereby triggering abdominal pain.

Thus, many patients—particularly those with duodenal bulb ulcers—commonly experience abdominal pain at night, a symptom known as “nocturnal pain.” Clinically, the presence of nocturnal pain warrants heightened vigilance, as it often indicates an underlying organic disease.

Of course, nocturnal pain may also arise from conditions other than duodenal bulb ulcer—for example, pancreatic cancer can likewise present with nocturnal pain. Therefore, we emphasize that anyone experiencing nocturnal pain should promptly seek medical evaluation to avoid missed or delayed diagnosis.
