What is commonly referred to as “acne inside the ear” is medically termed an external auditory canal furuncle. This condition represents a localized suppurative inflammation of the skin lining the external auditory canal. The outer one-third of the external auditory canal contains numerous hair follicles, sebaceous glands, and ceruminous glands. When bacteria infect these structures, localized erythema or abscess formation occurs. Primary symptoms include severe, throbbing otalgia—often exacerbated by jaw movement (e.g., during chewing or mouth opening)—and frequently radiating to the head. Systemic manifestations such as malaise or low-grade fever may also occur. If the furuncle enlarges sufficiently to obstruct the external auditory canal, conductive hearing loss may result; symptom relief typically follows spontaneous rupture and drainage of the lesion. Treatment includes topical application of 1% or 2% phenol glycerin ear drops to reduce inflammation and alleviate pain. Local heat application and physical therapy may also be beneficial. For fluctuant furuncles, incision and drainage are indicated. In cases of recurrent furunculosis, underlying chronic systemic conditions—including diabetes mellitus, nephritis, or malnutrition—should be investigated and managed accordingly.

1. Excess Internal Heat (“Shang Huo”)

Acne developing either in the external or internal ear may stem from excessive internal heat—a traditional Chinese medicine concept. A dense network of lymphatic vessels encircles the jawline and ears. As lymphatic tissue plays a key role in detoxification, excess internal heat may trigger inflammatory responses—manifesting as acne on the ears, lateral face, or jaw—as part of the body’s natural toxin-elimination process.

2. Kidney Deficiency

Excessive work-related stress or irregular sleep patterns may lead to kidney deficiency, resulting in ascending deficient fire—a TCM pattern associated with internal heat and inflammation. This imbalance may manifest as ear acne. Affected individuals should consider short-term anti-inflammatory medication, increase fluid intake, avoid late-night activities, ensure adequate rest, and refrain from consuming spicy, sour, or stimulating foods—including seafood and coriander (cilantro)—to effectively alleviate symptoms.
