Introduction
Occult blood refers to an increase in red blood cells detected during routine urinalysis. In a small number of cases, this is termed “urinary occult blood,” and on the routine urinalysis report, it is indicated by “+” or “−” under the “urinary red blood cells” category. Therefore, a positive urinary occult blood result suggests that the individual may have conditions such as urinary tract tumors, nephritis, or infections.

A positive urine occult blood test indicates the presence of hemoglobin or myoglobin in the urine, resulting in a positive test result during routine urinalysis—suggesting that red blood cells may be present in the urine. In fact, a positive occult blood finding warrants further diagnostic evaluation to determine the underlying cause. So, what does a positive urine occult blood test signify? The following section addresses this question.

What Does a Positive Urine Occult Blood Test Mean?

Occult blood refers to an increased number of red blood cells detected in routine urinalysis. When only a small number of red blood cells are present, it is termed “urine occult blood.” On standard urinalysis reports, the red blood cell count is typically indicated by “+” (positive) or “–” (negative). Under normal conditions—i.e., when red blood cells are absent or within the reference range—the red blood cell field reads “negative” (–). Conversely, when red blood cells are elevated—indicating occult blood—the result is reported as “positive” (+). Therefore, a positive urine occult blood test may suggest underlying conditions such as urinary tract tumors, glomerulonephritis, or urinary tract infections. However, isolated occult blood positivity alone is insufficient for definitive diagnosis, as even healthy individuals may occasionally exhibit transient occult blood; in such cases, however, the test remains negative under normal physiological conditions.

Knowledge Extension: Potential Risks Associated with Positive Urine Occult Blood

1. Risk of Infection Transmission

When one partner is infected, the other partner’s risk of contracting the same infection may reach approximately 70%. Moreover, family members who come into indirect contact with the patient’s bedding, towels, or bathtubs face an infection risk of about 40%.

2. Risk of Inflammatory Progression

Although local symptoms of genitourinary infections may temporarily subside or resolve within a certain timeframe and scope, this does not necessarily indicate complete recovery. Instead, the condition may have transitioned from acute to chronic, with pathological changes progressing deeper into surrounding tissues.

The above provides an overview of what a positive urine occult blood test signifies. We hope this information is helpful to you.
