In adolescents, primary dysmenorrhea is common and typically resolves spontaneously; by contrast, secondary dysmenorrhea—occurring after menarche—is usually associated with underlying pathology and requires timely medical evaluation. Analgesics alone should not be relied upon to manage such pain. This is one of the most common causes of lower abdominal pain.

2. Ovulation-related follicular rupture

During the ovulatory phase, some women experience a sensation of distension or dull pain in the lower abdomen due to follicular rupture. Generally, this type of pain is benign and carries no adverse health consequences. However, in rare cases, a large follicular tear may damage small blood vessels, leading to ovarian rupture and intraperitoneal hemorrhage—a gynecological emergency requiring immediate hospital care.

3. Gynecological inflammation

Right lower abdominal pain in women is frequently attributable to gynecological conditions, with inflammatory disorders being the most common—such as acute or chronic pelvic inflammatory disease (PID), salpingitis, or oophoritis (adnexitis). Pain associated with these conditions is typically characterized by a dragging or downward-pulling sensation, often accompanied by fever and abnormal vaginal discharge.

4. Appendicitis

Pain initially begins in the upper abdomen and gradually migrates to the right lower quadrant. Additional symptoms include nausea, vomiting, anorexia, fever, and tenderness on palpation of the right lower abdomen. This condition constitutes a surgical emergency requiring urgent hospital evaluation. Some individuals may feel perfectly well one moment, then suddenly develop severe abdominal pain accompanied by nausea and vomiting—highly suggestive of acute appendicitis.
