Rhinoplasty using the “dragon ratio” technique corrects a low nasal bridge by implanting filler material at the depressed area of the nose to elevate the nasal bridge to an appropriate height, thereby achieving an ideal nasal contour. From the perspective of nasal development, rhinoplasty for a low nasal bridge is best performed after puberty—typically after age 17—because the nasal bones and cartilage have largely completed their development by then.

The height of the nose is primarily determined by genetics. Most nasal abnormalities are congenital in origin and typically require rhinoplasty (nose augmentation surgery) for correction. A low nasal bridge may impair respiratory function—particularly its ability to act as a barrier against bacterial entry—and also adversely affect nasal aesthetics. Therefore, augmentation of a low nasal bridge can be achieved via injection of materials such as silicone gel into the nasal area.

What Can Be Done for a Low Nasal Bridge?

A flat or underdeveloped nose can be corrected through cosmetic surgery, with options including surgical rhinoplasty and non-surgical, minimally invasive injectable procedures.

Rhinoplasty for correcting a low nasal bridge generally involves implanting biocompatible materials at the depressed areas of the nose to elevate the nasal bridge to an aesthetically appropriate height, thereby achieving a more desirable nasal contour. From a developmental perspective, it is advisable to perform this procedure after completion of puberty—typically after age 17—since nasal bone and cartilage development is largely complete by then. Performing surgery at this stage minimizes the risk of postoperative changes or secondary nasal deformities.

Flat nasal bridges are classified into three degrees of severity: mild, moderate, and severe.

Mild: Slight depression of the nasal bones; the nasal dorsum appears slightly flattened and broad, with minor concavity or an unnatural curvature.

Moderate: Noticeable depression of the nasal bridge; wide nasal root; upwardly oriented nostrils (“skyward” appearance); low and flattened nasal tip and alar cartilages; shortened overall nasal length.

Severe: Marked dorsal depression; poorly defined nasal bridge; often accompanied by structural deformities involving the external nasal skeleton, nasal septum, and nasal mucosa.

How Can a Flat Nasal Bridge Be Corrected?

Autologous tissue grafting (e.g., cartilage) involves harvesting cartilage from another site in the body—such as the ear, nasal septum, or rib—and sculpting it to fit the desired nasal contour before implantation into the nasal bridge region. This technique requires two surgical sites: one for donor tissue harvest and another for nasal implantation.

Silicone implant rhinoplasty is technically straightforward; however, the resulting nasal bridge may feel firm or unnatural. In rare cases, patients may experience foreign-body reactions or implant rejection. Silicone implants are relatively inexpensive and require less technical expertise, making them widely accessible to most surgeons.

Expanded polytetrafluoroethylene (ePTFE, commonly known as “Gore-Tex”) is currently considered one of the safest and most effective synthetic implant materials, with no reported adverse effects. However, it is significantly more expensive and demands greater surgical skill. Widely used in international aesthetic surgery, ePTFE is highly stable, chemically inert, non-toxic, non-carcinogenic, and non-teratogenic. Its soft texture facilitates precise carving and natural integration.

Injectable rhinoplasty operates on a principle similar to surgical augmentation—both aim to elevate the nasal bridge by introducing volume into the nasal tissues. The key distinction lies in delivery method: injectable rhinoplasty uses fine-needle injections without incisions, whereas surgical rhinoplasty requires skin incisions and direct implant placement.
