Is induced abortion painful?

A small proportion of women cannot tolerate the pain caused by induced abortion. If the pain is truly unbearable, anesthesia is required.

“Induced abortion” (often shortened to “abortion” or “IA”) refers to a medical procedure used to terminate pregnancy. Generally, induced abortion surgery is painful; however, because individual pain tolerance varies significantly, each woman’s perception of the pain associated with the procedure also differs. Approximately 80% of women can tolerate the pain from induced abortion surgery, whereas about 20% find it intolerable.

The pain associated with induced abortion primarily occurs in two phases: First, pain during cervical dilation—the most intense phase of discomfort. Second, pain during uterine aspiration and curettage—the sensation is typically dull and tolerable for most women.

Therefore, for women particularly anxious about pain during induced abortion, pretreatment with mifepristone and misoprostol prior to the procedure may be considered to soften the cervix. This significantly reduces procedural pain, thereby improving the patient’s ability to tolerate the procedure.
