Hepatobiliary duct drainage can be performed either surgically or via interventional procedures, and may involve either internal or external drainage. This procedure is indicated for patients with: stenosis of the common hepatic duct or left hepatic duct; long-segment stenosis involving both right and left hepatic ducts, where surgical incision of the stenotic segment is technically challenging; isolated stenosis of secondary (segmental) hepatic ducts; or stenosis of the right posterior hepatic duct.

What Is Bile Drainage Surgery?

Bile drainage surgery refers to hepatobiliary duct drainage, a procedure that significantly improves clinical outcomes in patients with conditions such as hepatic duct stenosis.

Hepatobiliary duct drainage can be performed either surgically or via interventional (minimally invasive) techniques and may involve either internal or external drainage. It is indicated for patients with common hepatic duct or left hepatic duct stenosis; those with long-segment stenosis involving both right and left hepatic ducts, where surgical incision of the stricture is technically challenging; patients with isolated secondary (segmental) hepatic duct stenosis; and those with stenosis of the right posterior hepatic duct.

Bile drainage surgery is typically performed under general anesthesia or epidural anesthesia. Preoperatively, antibiotics—such as cefuroxime sodium for injection—are administered to prevent and control pulmonary or biliary tract infections. Postoperative care is critical to prevent complications including bile leakage, biliary tract infection, and biliary hemorrhage.

Postoperatively, patients may gradually advance their diet from liquids to regular meals, selecting low-fat, high-protein, easily digestible foods. Patients should avoid physical overexertion and ensure adequate sleep. A semi-recumbent position is recommended postoperatively to facilitate drainage and reduce infection risk. Early, moderate ambulation is also encouraged.
