Therefore, treatment primarily focuses on symptomatic and etiological management—tailoring interventions to the individual patient’s specific condition and underlying cause, thereby implementing personalized therapy. How to Effectively Treat Hepatic Ascites
Diuretic therapy remains the cornerstone of treatment for hepatic ascites, regardless of severity. For patients with mild ascites, oral diuretics are typically prescribed. Commonly used agents include spironolactone and hydrochlorothiazide.

In cases of moderate-to-severe ascites where oral diuretics alone prove insufficient, intravenous diuretics may be added. Frequently used intravenous agents include furosemide injection and torasemide—both highly effective diuretics.
During the decompensated phase of end-stage cirrhosis—particularly when severe ascites develops and oral or intravenous diuretics yield suboptimal results—paracentesis (therapeutic abdominal paracentesis) is recommended, irrespective of the patient’s clinical status. This procedure effectively alleviates ascites-related symptoms, including dyspnea and marked abdominal distension.
Notably, the volume of ascitic fluid removed during paracentesis must be carefully controlled. During the first paracentesis, fluid removal should not exceed 1,000 mL to minimize the risk of adverse events. In subsequent intermittent paracenteses, the volume may be cautiously increased based on the patient’s clinical status, tolerance, and hepatic function—but generally should not exceed 3,000 mL.
