There are two primary clinical causes of lower back pain upon waking. First, exposure to cold during sleep—such as sleeping uncovered or being exposed to air conditioning or fans—can lead to a “cold invasion” (a traditional Chinese medicine concept), causing constriction of capillaries in the lumbar region. This reduces local blood supply and results in relative ischemia and hypoxia of the lumbar muscles, thereby triggering dull, aching pain in the lower back. Second, ankylosing spondylitis—a chronic inflammatory disease affecting the spine and sacroiliac joints—may cause morning lower back pain, particularly in young and middle-aged adults.

To relieve lower back pain upon waking, avoid poor posture, apply local heat therapy (e.g., using a hot water bottle or infrared lamp), and minimize activities involving bending, heavy lifting, or prolonged sitting. If pain persists, topical nonsteroidal anti-inflammatory drugs (NSAIDs) such as Voltaren gel or Yunnan Baiyao spray may be used.
