Introduction
Facial acne caused by Demodex mites may be treated with metronidazole gel or 10% sulfur ointment. If the acne lesions are red, swollen, warm, and inflamed, an anti-inflammatory topical agent such as erythromycin ointment may be considered. Metronidazole gel helps alleviate local inflammatory reactions induced by Demodex mite infection and exerts a scabicidal effect against the mites. As a topical medication, it is effective for acne associated with Demodex infestation.

In general, acne caused by facial mites may be treated with topical agents such as metronidazole gel or 10% sulfur ointment. If the acne lesions are red, swollen, warm, and inflamed, an anti-inflammatory topical medication—such as erythromycin ointment—may be considered.

1. Metronidazole Gel: Metronidazole gel helps reduce local inflammatory reactions induced by *Sarcoptes scabiei* (scabies mites) infection and kills mite organisms. It is a topical agent effective against mite-related acne. Rare adverse effects include erythema, skin dryness, and local skin irritation. Due to its potential teratogenicity, metronidazole gel is contraindicated in pregnant and breastfeeding women.

2. 10% Sulfur Ointment: This oil-based preparation possesses antimicrobial, antiparasitic, and keratolytic properties. It exerts antibacterial and anti-inflammatory effects, inhibits fungal growth, eradicates cutaneous parasites, dissolves hyperkeratotic stratum corneum, and reduces sebum overproduction. Additionally, it provides effective pruritus relief and skin lubrication. It is clinically beneficial for bacterial and fungal infections, disorders of keratinization, and diseases involving skin appendages. When applied topically, it also promotes follicular opening, thereby enhancing its efficacy against scabies mites and other dermatologic mites.

3. Erythromycin: Erythromycin is a bacteriostatic antibiotic with potent activity against Gram-positive bacteria—including *Staphylococcus aureus*, *Streptococcus pyogenes*, *Streptococcus pneumoniae*, *Corynebacterium diphtheriae*, *Bacillus anthracis*, and *Clostridium* species. It is highly effective for controlling inflammation and bacterial proliferation.
