Introduction
Causes of cracking and itching on the inner side of the toes include: 1. Fungal infection: In some patients, cracking occurs on the skin between or around the toes. 2. Skin allergy: Some patients may develop fissures on the skin behind the toes. 3. Hereditary factors: “Toe splitting” in this context refers to nail splitting (onychoschizia). 4. Traumatic toenail injury: Nail splitting caused by physical trauma to the toenail.

Toe fissures (cracks) are primarily caused by fungal infections, skin allergies, genetic traits, and traumatic toenail injuries. Specific causes are outlined below:

1. Fungal infection: In some patients, “toe cracking” refers to fissuring of the skin on the toes. When skin fissures occur between the toes, the most common cause is a fungal infection—specifically, tinea pedis (athlete’s foot).

2. Skin allergy: Some patients may develop skin fissures on the posterior aspect of the toes or other areas. This may indicate eczema—a condition triggered by skin allergy—and often persists chronically at localized sites. Marked dryness and cracking typically follow hyperkeratosis (excessive thickening of the outer skin layer).

3. Genetic trait: In certain individuals, “toe cracking” refers to longitudinal splitting or fissuring of the toenail itself. If such nail fissuring occurs specifically in the little toe and has persisted for years without associated symptoms or discomfort, it is generally not pathological but rather a benign inherited trait.

4. Traumatic toenail injury: Toe fissures may also result from trauma to the toenail—for example, due to impact or compression injuries.

If fissures develop on the inner side of the toes accompanied by itching, attention should be paid to foot hygiene and proper foot care, and healthy daily habits should be cultivated.
