Renal cell carcinoma (RCC) is a malignant tumor originating from the epithelial cells of the renal tubules in the kidney parenchyma. It is typically asymptomatic in its early stages; however, in advanced stages, a minority of patients may present with hematuria, an abdominal mass, or flank pain. The precise etiology remains unclear but may involve factors such as obesity, smoking, and genetic predisposition. Prompt treatment is essential upon diagnosis.

How Is Advanced-Stage Renal Cell Carcinoma Treated?

1. Pharmacotherapy

Commonly used targeted agents include sorafenib, axitinib, sunitinib, bevacizumab, lenvatinib, pazopanib, temsirolimus, and everolimus. These agents are also indicated for unresectable clear-cell renal cell carcinoma. Efficacy varies among drugs, and combination regimens are often required to achieve optimal therapeutic outcomes.

2. Ablative Therapy

Ablative therapy is another option for patients with advanced RCC. Under image guidance—such as ultrasound, CT, or MRI—the tumor is precisely localized, and physical or chemical methods are then applied directly to destroy the tumor tissue. Common ablative modalities include radiofrequency ablation and cryoablation. Studies show no significant differences between these two techniques regarding overall survival, cancer-specific survival, recurrence-free survival, or complication rates.

3. Comprehensive (Multimodal) Therapy

For patients with advanced RCC, a multimodal treatment approach is often adopted. In selected patients with good performance status and distant metastases, palliative nephrectomy may be performed to enhance the efficacy of subsequent systemic drug therapy.
