First, to achieve conception, sexual intercourse must occur to deposit the male’s semen into the female’s vagina, and the female must have normal ovulatory function. Around the midpoint of the menstrual cycle, a single mature oocyte is released; the optimal time for fertilization of this mature oocyte is within 12–24 hours after ovulation. Second, the female’s fallopian tubes must exhibit normal peristaltic activity and maintain patency of the tubal lumen.

Many newly married women are planning to conceive but have not yet become pregnant, leading to significant anxiety. How do women become pregnant?

How Do Women Become Pregnant?

First, for conception to occur, sexual intercourse must take place, allowing the male’s semen to be deposited into the female’s vagina. The woman must also have normal ovulatory function. Around mid-cycle—approximately day 14 of a typical 28-day menstrual cycle—one mature egg is released from the ovary. The optimal window for fertilization of this mature egg is within 12–24 hours after ovulation.

Second, the woman’s fallopian tubes must function properly, exhibiting coordinated muscular contractions (peristalsis) and maintaining patency of the tubal lumen. Under these conditions, cilia lining the fallopian tubes “capture” the mature egg, while peristaltic movements and ciliary beating transport the egg gradually toward the uterine cavity.

Simultaneously, both the fallopian tubes and the uterine cavity must remain fully patent—free of any obstruction. During this time, sperm from the male’s ejaculate progressively enter the fallopian tube lumen and await the arrival of the egg. Only when sperm and egg successfully meet can fertilization occur. Approximately six days after fertilization, the resulting blastocyst implants into the endometrium. Trophoblast cells begin to form, and human chorionic gonadotropin (hCG) is secreted—marking the onset of pregnancy, which can then be detected.
