How to Treat Winter-Related Itching of the Lower Legs
Elderly individuals or those with dry skin often experience reduced secretory function of the sweat and sebaceous glands in the lower legs, diminished stratum corneum hydration capacity, and decreased levels of the skin’s natural water-lipid emulsion—leading to dryness and loss of skin luster. In patients with dry-skin-associated pruritus, excessive bathing and scalding with hot water can significantly worsen both dryness and itching. This condition commonly occurs in winter, when low ambient humidity further exacerbates lower-leg pruritus. Typically, the anterior (extensor) surface of the lower legs is affected, presenting with characteristic features including dry skin, fine fissures, scaling, and moderate-to-severe pruritus. Secondary excoriations, crusts, and scratch marks often develop due to repeated scratching.

Chronic venous insufficiency leading to stasis dermatitis may initially manifest as eczematous lesions on the anterior shins, bilateral lower legs, or around the ankles—often accompanied by marked pruritus. Winter’s dry climate can further aggravate pruritus associated with stasis dermatitis. Additionally, some patients may have elevated blood glucose levels or diabetes mellitus, which themselves can cause lower-leg pruritus. Patients should aim to preserve the minimal natural sebum layer on the lower legs: avoid scalding hot water, refrain from over-washing with soap or shower gels, and avoid scratching or rubbing. Regular application of moisturizing emollients—such as urea cream—is strongly recommended.
