The primary effect and benefit of acyclovir cream is inhibition of herpes simplex virus (HSV) and varicella-zoster virus (VZV). Acyclovir is a broad-spectrum antiviral agent that selectively inhibits and inactivates viral DNA polymerase, thereby blocking viral DNA synthesis. It exhibits potent antiviral activity against both HSV and VZV. Acyclovir ointment is a topical formulation clinically used primarily to treat herpes simplex, genital herpes, and herpes zoster (shingles); it is also indicated for varicella (chickenpox).

Application involves applying a small amount of the ointment directly onto the shingles rash. If a herpes patient develops a concurrent bacterial infection, antibiotic therapy should be administered alongside acyclovir ointment to manage secondary bacterial infection. Therefore, acyclovir ointment is indicated specifically for treating viral skin diseases. It is primarily used for cutaneous and mucosal infections caused by herpesviruses.

Because only a small area is typically treated and systemic absorption is minimal, gastrointestinal disturbances, hepatic or renal dysfunction—commonly associated with intravenous or oral acyclovir—are rarely observed. Similarly, adverse effects such as numbness or tingling in the extremities, tremors, or generalized fatigue are uncommon. However, local application may cause side effects at the site of administration, including pruritus, erythema, and swelling.
