Salpingitis, in particular, can cause tubal adhesions and obstruction, leading to fallopian tube blockage. Tubal patency is crucial for conception: during normal conception, sperm travel through the vagina, cervix, and uterine cavity, ultimately meeting the egg and fertilizing it in the ampulla of the fallopian tube.

If the fallopian tubes are obstructed or partially blocked, this may result in infertility or ectopic pregnancy. Generally speaking, PID can cause infertility—but not all cases of PID lead to infertility. Mild PID may not alter tubal anatomy and thus may not impair fertility. Therefore, infertility is more commonly associated with severe PID—particularly cases that distort tubal structure.
