During the ataxia phase, individuals often experience euphoria and reduced coordination, increasing susceptibility to injury—particularly to the lower limbs—as well as to excessive force or stretching applied to the feet. For instance, trauma is a frequent cause of lower-limb weakness after drinking.

After alcohol consumption, ethanol is metabolized in the liver by alcohol dehydrogenase into acetaldehyde. Subsequent breakdown of acetaldehyde releases lactic acid, which tends to accumulate in skeletal muscle tissue, leading to feelings of leg weakness. This accumulation is especially pronounced in regions with highly developed skeletal muscles and high muscle density—namely, the thighs and calves—where lactic acid buildup is most noticeable. Further metabolism of lactic acid produces acetic acid, which can exacerbate subjective sensations of bodily pain.
