In general, kissing does not transmit hepatitis B. For hepatitis B infection to occur, three essential conditions of infectious disease transmission must all be present: a source of infection (i.e., an infected individual), a route of transmission, and a susceptible host—specifically, a person lacking protective antibodies against HBV.

Secondly, while kissing involves only minimal exchange of bodily fluids, under special circumstances—such as when the infected individual is “HBsAg-positive, HBeAg-positive, and anti-HBc-positive” (commonly termed “big three positive”)—their saliva may contain detectable levels of HBV. If the uninfected partner has oral mucosal breaks or ulcers during kissing, transmission of HBV is theoretically possible—but the likelihood remains extremely low. To better prevent HBV infection, timely vaccination against hepatitis B is essential; once sufficient protective antibodies have developed in the body, infection will not occur.
