For patients with phimosis or redundant prepuce, circumcision may be performed; following surgery, gradual desensitization of the glans occurs through repeated friction against underwear. If preputial length is not excessive—or if hypersensitivity persists despite circumcision—topical application of lidocaine cream to the glans prior to intercourse may help reduce sensitivity. Another option is surgical intervention, specifically dorsal penile nerve block surgery, which can decrease glans sensitivity. However, this is an invasive procedure and is not suitable for everyone—particularly young patients are generally discouraged from undergoing it, and surgical treatment should be approached with great caution.

To reduce glans sensitivity, targeted exercises for the glans may be helpful. Regardless of physiological state, repeatedly and consistently gently pinching or massaging the glans with fingers can enhance neural and vascular responsiveness, thereby effectively improving sexual performance. Another enjoyable method is “shower massage” of the glans: using a showerhead to direct warm water onto the glans tip and surrounding base (with the foreskin retracted to fully expose the glans) provides gentle yet effective stimulation. High-pressure, concentrated warm water applied directly to acupoints may activate ligaments and nerves essential for erectile support. Alternating between warm and cool water during localized showering enhances this effect even further.
