The specific symptoms of infantile cerebral palsy include: delayed motor development and reduced spontaneous movement. A delay of more than three months beyond expected developmental milestones is generally considered abnormal, given individual variations in growth. Therefore, familiarity with key developmental milestones during infancy and early childhood is crucial for accurate clinical assessment. Abnormal muscle tone—manifesting differently across CP subtypes—is another hallmark feature. In spastic CP, muscle tone is increased; in hypotonic CP, muscle tone is decreased, resulting in flaccid paralysis and limb weakness—though deep tendon reflexes may still be elicitable. In dyskinetic CP, muscle tone fluctuates unpredictably (dystonia). Abnormal postures are closely associated with disturbances in muscle tone—for example, a scissoring or opisthotonic posture in cases of hypertonia, or a frog-leg posture in cases of hypotonia.

Abnormal reflexes constitute an important neurological sign in diagnosing CP. These include persistence of primitive reflexes beyond their typical age of disappearance, as well as absence or delay in the emergence of normal physiological and postural reflexes. Children with CP may also present with comorbidities such as intellectual disability, epilepsy, behavioral abnormalities, visual or auditory impairments, and sensory deficits.

Abnormal brain development during the embryonic period—due to maternal trauma or severe infection, diabetes, or exposure to ionizing radiation during pregnancy—can cause permanent brain injury in the fetus. Similarly, maternal infection with viruses such as rubella or varicella-zoster during early pregnancy may damage the fetal central nervous system and lead to neonatal cerebral palsy.
