Advanced gastric cancer is a malignant gastric tumor representing a specific stage in the progression of gastric cancer—encompassing both mid-stage and certain cases of locally advanced gastric cancer. At this stage, cancer cells have infiltrated the muscularis propria and serosal layers of the stomach. Lymph node metastasis and/or distant metastasis may be present, although distant metastasis is not always observed. Regardless of whether metastasis has occurred, prompt and aggressive treatment is essential; otherwise, the patient’s condition may deteriorate rapidly within a short time, potentially leading to widespread systemic metastasis and life-threatening complications.

Patients with advanced gastric cancer commonly experience abdominal pain, abdominal distension, nausea, anorexia, fatigue, weight loss, anemia, and hematochezia. Disease progression is typically rapid. Endoscopically, diffuse lesions are frequently observed—including large, deep ulcers covered with thick white exudate, surrounded by marked hyperemia, edema, and erosion—or large, bulky tumorous lesions. Biopsy specimens often bleed easily; mucosal folds around the lesion are disrupted, gastric wall rigidity is evident, and peristalsis is significantly impaired. Histopathologically, infiltrative carcinoma or signet-ring cell carcinoma is commonly identified.

Treatment options include surgical resection and/or radiochemotherapy; however, long-term prognosis remains poor.
