After insertion of an intrauterine device (IUD), many adverse reactions may occur. Some women experience lower back pain, while a small number may have vaginal bleeding. What symptoms indicate IUD displacement?
Symptoms of IUD Displacement
Pregnancy is one possible symptom of IUD displacement. Once displaced, the IUD loses its contraceptive efficacy; thus, engaging in unprotected intercourse significantly increases the risk of pregnancy. Pain is another common symptom—patients often report lower abdominal distension or discomfort and lower back pain, which may be mistakenly attributed to dysmenorrhea, thereby delaying diagnosis and treatment. Abnormal vaginal discharge may also occur: some patients notice increased discharge, sometimes with blood-tinged mucus; in severe cases, vaginal bleeding may develop. Menstrual irregularities are also common, including markedly heavier menstrual flow, prolonged menstruation, or delayed periods.

An intrauterine device functions by preventing implantation of a fertilized egg. Clinically, it is the most commonly used contraceptive method for postpartum women. IUD displacement may present with mild abdominal pain, lower back stiffness, or slight vaginal bleeding; however, some women remain entirely asymptomatic. Importantly, a displaced IUD no longer provides effective contraception, and unprotected intercourse during the fertile window may result in pregnancy. Ultrasound examinations are recommended at 1, 3, 6, and 12 months after IUD insertion, followed by annual follow-up ultrasounds thereafter to monitor for IUD expulsion or downward migration.
