Cystic degeneration of uterine leiomyomas is a type of leiomyoma degeneration characterized primarily by loss of the tumor’s typical histological features, with necrosis and liquefaction of smooth muscle cells. It most commonly evolves from hyaline degeneration. In such cases, the leiomyoma becomes softer, making differentiation from a pregnant uterus or ovarian cyst more challenging. Within cystically degenerated leiomyomas, cystic cavities of varying sizes develop, separated by connective tissue; these cavities contain clear fluid that may coagulate into a gelatinous substance. Microscopically, the inner walls of these cystic cavities lack epithelial lining.
