Sharp chest pain during deep breathing may be caused by various factors. This condition is sometimes due to pleuritis, an inflammation of the pleura—the membranes lining the lungs and chest cavity—which can cause sharp pain when the inflamed layers are stretched during deep breaths.

Pericarditis may also be a contributing factor; inflammation of the pericardium causes the membrane surrounding the heart to rub against itself during respiration, resulting in sharp pain in the cardiac area. Intercostal neuralgia can produce similar symptoms—when nerves are irritated or damaged, deep breathing may intensify the pain, leading individuals to mistakenly believe the pain originates from the heart.

Cardiac conditions such as coronary artery disease may also cause pain during deep breathing, especially when there is insufficient blood supply to the heart muscle. If this stabbing pain occurs frequently, persists, or is accompanied by other symptoms such as shortness of breath, palpitations, or chest tightness, prompt medical evaluation is recommended.
