A low red cell distribution width (RDW) may be caused by various factors.

Physiological causes include blood dilution due to excessive water intake in a short period, which usually does not require treatment. Pathological causes are often related to anemia, such as iron deficiency anemia, megaloblastic anemia, and aplastic anemia. These conditions may lead to bone marrow failure in blood cell production and a reduction in overall blood cell counts, resulting in a decreased RDW.

Patients may experience symptoms such as pale complexion, fatigue, dizziness, and even palpitations and shortness of breath. Treatment may involve dietary adjustments to increase iron intake, or taking medications as prescribed by a physician. In severe cases, blood transfusion may be considered.
