How to treat a 2cm kidney stone?

I just completed a physical examination and found out that I have a kidney stone that has reached 2 cm in size. How should a 2 cm kidney stone be treated?
For a 2cm renal calculus, the selection of treatment options should be determined comprehensively based on the specific location and number of the stone, the patient's physical condition, and the physician's recommendations.
1. Observation and conservative treatment: If there are no obvious symptoms and no urinary tract obstruction, spontaneous passage may be attempted through increased fluid intake and physical activity. Stone-expelling medications may also be used as an adjunct.
2. Extracorporeal shock wave lithotripsy (ESWL): This may be considered for multiple stones that are in relatively favorable positions, breaking the stones into smaller fragments that can be passed in the urine. However, this method may not be suitable for all 2cm stones.
3. Surgical treatment:
- Percutaneous nephrolithotomy (PCNL): A tract is created from the skin to the kidney, through which stones are fragmented using laser or ultrasonic energy and then removed. This procedure is suitable for larger or more complex stones.
- Flexible ureteroscopic lithotripsy: A flexible ureteroscope is passed through the natural urinary pathways to reach the kidney, resulting in less renal trauma. However, the procedure may be technically more complex and may require multiple sessions to completely clear the stones.
