Blood in the urine during menstruation can be caused by multiple factors.

1. Mild hematuria occasionally occurring before or after menstruation may result from pelvic congestion, pressure on or minor injury to the urethral opening during the menstrual period, and generally does not require excessive concern.

2. Contamination of urine by menstrual blood due to shedding of the uterine lining during menstruation is a normal physiological phenomenon and does not require specific treatment.

3. Poor personal hygiene during menstruation may allow bacteria to enter the urethra, causing urethritis or cystitis, which can lead to hematuria. In such cases, patients should seek medical attention promptly and may be treated with medications such as levofloxacin or moxifloxacin.

4. If hematuria is accompanied by pain, frequent urination, urgency, or persists for a prolonged period, urinary system diseases such as urinary tract infections, stones, or tumors should be considered.

Gynecological conditions, such as uterine fibroids or cervical inflammation, may also affect urine color. Additionally, kidney problems, including nephritis and abnormal renal vasculature, should not be overlooked. Therefore, patients are advised to seek timely medical evaluation, including urinalysis and B-ultrasound, to identify the underlying cause and receive targeted treatment. Maintaining good personal hygiene and avoiding excessive fatigue and emotional stress are also recommended.
