The health of the kidneys directly affects the abundance or deficiency of essence (Jing), Qi, and blood. Once kidney deficiency occurs, the body manifests a series of symptoms. Particularly in men, poor kidney function often correlates with diminished sexual function. Kidney deficiency is further classified into two main patterns: Kidney Yang Deficiency and Kidney Yin Deficiency. So how can one distinguish between Kidney Yin Deficiency and Kidney Yang Deficiency?
How to Differentiate Kidney Yin Deficiency from Kidney Yang Deficiency
Although both Kidney Yang Deficiency and Kidney Yin Deficiency stem from the same underlying pathological condition—kidney deficiency—they present with distinct clinical manifestations and require different therapeutic approaches. Kidney Yin Deficiency typically leads to symptoms associated with Yin deficiency and relative excess of internal heat (fire). These may include soreness and weakness in the lower back and knees, tinnitus, dizziness, insomnia, and vivid dreams. In women, menstrual flow may become scanty or cease entirely (amenorrhea); some patients may experience abnormal uterine bleeding (metrorrhagia or menometrorrhagia). Men may exhibit hypersexuality, premature or spontaneous erections, nocturnal emissions, or premature ejaculation. Both men and women commonly experience tidal fever, night sweats, a sensation of heat in the palms, soles, and chest (“five-center heat”), dry throat, and thirst. Treatment focuses primarily on nourishing Yin and tonifying the kidneys, using formulas such as Liu Wei Di Huang Wan (Rehmannia Six Formula) or Zhi Bai Di Huang Wan (Anemarrhena–Phellodendron Rehmannia Formula).

Kidney Yang Deficiency differs significantly from Kidney Yin Deficiency. Patients with Yang Deficiency mainly present with symptoms dominated by cold and deficiency—such as lassitude and weakness in the lower back and knees, tinnitus, and cold pain in the lower limbs. Their complexion may appear dull or sallow, and they often feel fatigued and listless. Men may experience diminished libido (hyposexuality), while women may not only suffer reduced sexual desire but also develop infertility and related reproductive disorders. Therapeutically, formulas such as You Gui Wan (Right-Returning Pill) or Jin Kui Shen Qi Wan (Golden Cabinet Kidney Qi Pill) are commonly prescribed.
