Introduction
Diarrhea after taking fosfomycin trometamol powder is one of the common adverse reactions of this medication. Additionally, since food affects the absorption of fosfomycin trometamol powder, it is recommended to separate the time of administration from meal times—take the medication either 2–3 hours before or after eating. To reduce gastric mucosal irritation, avoid consuming spicy, irritating, raw, cold, greasy foods and refrain from drinking alcohol during treatment.

Diarrhea after taking trometamol fosfomycin powder is one of the common adverse reactions of this medication.

Trometamol in trometamol fosfomycin powder is generally used to increase the water solubility of fosfomycin. However, some patients' gastrointestinal tracts are sensitive to trometamol, which may stimulate intestinal mucosal motility and increase secretions, leading to diarrhea as an adverse effect. Most individuals taking trometamol fosfomycin powder experience diarrhea to varying degrees, with the majority having mild cases—defined as fewer than four additional bowel movements per day compared to baseline. This mild form typically does not involve bloody stools or other significant discomfort and usually resolves spontaneously without specific treatment upon discontinuation of the drug.

If daily bowel movements increase by 4–6 times compared to baseline, it is considered moderate diarrhea; an increase of more than seven times indicates severe diarrhea. In cases of moderate to severe diarrhea, immediate discontinuation of the drug is recommended. Severe diarrhea not only impairs drug absorption but also causes significant discomfort, dehydration, and potentially life-threatening enteritis. Patients experiencing moderate to severe diarrhea should be treated with oral rehydration salts, lactase tablets, or similar agents to help regulate gastrointestinal function.

In addition, because food affects the absorption of trometamol fosfomycin powder, it is recommended to separate medication intake from meals by 2–3 hours—taking the drug either 2–3 hours before or after eating.
