Platelets are small cytoplasmic fragments shed from the cytoplasm of mature megakaryocytes in the bone marrow. Although megakaryocytes constitute the smallest proportion—only 0.05%—of nucleated cells in the bone marrow, the platelets they produce play a critical role in hemostasis. Causes of Thrombocytopenia

1. Hematologic Disorders

Thrombocytopenia may result from hematologic disorders, such as myelodysplastic syndromes (MDS) and aplastic anemia. Additionally, malignancies can infiltrate the bone marrow, leading to reduced platelet production. Autoimmune and rheumatic diseases may also trigger antigen-antibody reactions that destroy platelets, thereby causing thrombocytopenia.

2. Drug-Induced Causes

Certain medications can induce thrombocytopenia. Common examples include aspirin, anti-inflammatory and analgesic agents, and antithyroid drugs used by patients with hyperthyroidism. Therefore, medications should always be taken under the guidance of a physician.

3. Excessive Platelet Destruction

Thrombocytopenia may occur due to excessive platelet destruction. Platelet destruction is broadly categorized into congenital and acquired forms. Acquired excessive destruction further subdivides into immune-mediated and non-immune-mediated types. Congenital thrombocytopenia tends to be more challenging to treat, whereas acquired forms often respond well to appropriate therapeutic interventions.

4. Infections

Infections—including viral infections—can cause thrombocytopenia. Common viral agents include hepatitis viruses (e.g., HBV and HCV), Epstein-Barr virus (EBV), and cytomegalovirus (CMV). Genetic and environmental factors—such as prolonged exposure to radiation or chemotherapy—may also contribute to decreased platelet counts.
