What is the rubella virus? Biologically, the rubella virus belongs to the *Togaviridae* family and is classified within the genus *Rubivirus*. It is highly contagious, and the general population is susceptible. Infants and young children are particularly prone to infection. Infection during the first 20 weeks of pregnancy may result in miscarriage or congenital fetal abnormalities. The rubella virus has very low environmental stability outside the human body; it is heat-labile and sensitive to ultraviolet (UV) radiation, ether, and deoxycholic acid. It can be inactivated by heating at 56°C for 30 minutes.

Rubella virus is a single-stranded, positive-sense RNA virus belonging to the *Togaviridae* family and is a human-restricted pathogen. It readily causes vertical transmission; primary infection with rubella virus during early pregnancy enables the virus to cross the placental barrier and infect the fetus, often resulting in spontaneous abortion or stillbirth, and may lead to congenital rubella syndrome (CRS), causing fetal malformations. The virus exhibits poor viability outside the human host and is highly sensitive to ultraviolet (UV) light, cesium chloride, and deoxycholic acid.

What Is Rubella Virus?

Biologically, rubella virus belongs to the *Togaviridae* family and is classified within the arbovirus group. It is universally infectious among humans, with infants and young children being particularly susceptible. Infection during the first 20 weeks of pregnancy may result in miscarriage or fetal malformations. The virus demonstrates very low environmental stability and is highly sensitive to heat, UV radiation, ether, and deoxycholic acid. It can be effectively inactivated by heating at 56°C for 30 minutes.

Humans are the sole natural host for rubella virus. Transmission occurs primarily via respiratory droplets. The disease peaks in spring, with maximal infectivity occurring several days before, during, and after rash onset. The incubation period is relatively short; systemic and respiratory symptoms are generally mild, and fever typically precedes the appearance of rash by one to two days. In addition to nasopharyngeal secretions, the virus may also be detected in blood, feces, and urine. Rubella most commonly occurs during winter and spring, predominantly affecting children aged 1–5 years, with equal incidence in males and females. Maternal antibodies confer protection against clinical disease in infants up to six months of age.

The characteristic rash consists of pale red maculopapules that begin on the face and rapidly spread to involve the neck and trunk. The rash usually resolves within one to two days, without post-inflammatory hyperpigmentation or desquamation. However, it is frequently accompanied by tender enlargement of the postauricular and cervical lymph nodes.

Management and Prevention Measures for Rubella Virus Infection

1. Immediately isolate confirmed rubella cases; isolation should continue until five days after rash onset.

2. During rubella outbreaks, avoid taking susceptible children to crowded public places and prevent contact with infected individuals. Pregnant women—particularly those in the first two to three months of gestation—must strictly avoid exposure to rubella patients.

3. Ensure bed rest for affected children; avoid direct exposure to drafts to prevent secondary infections due to chilling, which may exacerbate illness. Encourage increased fluid intake during febrile periods. Diet should be light and easily digestible; fried and greasy foods should be avoided.

4. Prevent skin scratching to avoid secondary bacterial infection. The incubation period—from initial exposure to symptom onset—ranges from 14 to 21 days. A prodromal phase characterized by low-grade fever and nonspecific symptoms often goes unnoticed due to its mildness and brevity.
