Proper method for removing a PICC catheter

Introduction
The patient should be placed in a semi-recumbent position with the arm on the catheterized side abducted at a 45° angle. Gently peel back the edges of the dressing and remove it from the distal end to the proximal end of the catheter. Disinfect the skin within 15 cm around the catheter insertion site. Slowly withdraw the catheter outward from the puncture site parallel to the direction of the vein. For one month after catheter removal, the affected limb should not lift or raise heavy objects to avoid secondary injury.

Generally, a PICC catheter refers to a peripherally inserted central catheter, which is placed through a peripheral vein into the central venous system and is suitable for patients requiring repeated blood draws. Catheter removal must be performed by a trained professional. The specific procedure is as follows:

1. Perform vascular color Doppler ultrasound before removal to confirm the absence of wall-adherent thrombus around the PICC catheter.

2. Position the patient in a semi-recumbent position, abduct the arm on the catheterized side to a 45-degree angle, expose the insertion site, and place a treatment drape under the arm.

3. Gently peel back the edges of the dressing and remove it from the distal end toward the proximal end of the catheter. Disinfect the skin within a 15 cm radius around the insertion site.

4. Slowly withdraw the catheter outward along the direction parallel to the vein from the puncture site. After removal, inspect the catheter to ensure it is intact without breakage or damage.

5. Apply gauze locally and apply pressure for 1–2 minutes. Once significant bleeding is ruled out, cover the area with gauze and secure it with adhesive tape.

The limb used for catheter placement should avoid lifting or raising heavy objects for one month after removal to prevent secondary injury. Maintain a light diet and avoid spicy or irritating foods.
