Introduction
It is caused by fluctuations in hormone levels due to factors such as environmental changes, fatigue or stress, medication use, and significant emotional fluctuations, leading to severe damage and thinning of the endometrium, preventing normal endometrial shedding. Additionally, excessive secretion of androgens in the body inhibits follicular development and ovulation, resulting in the endometrium being exposed only to estrogen without the balancing effect of progesterone.

Absence of menstruation and pregnancy may be caused by physiological factors or pathological conditions, such as thin endometrium or polycystic ovary syndrome (PCOS). The specific analysis is as follows:

1. Physiological Causes

The absence of menstruation without pregnancy may result from environmental changes, fatigue or stress, medication use, or significant emotional fluctuations, which can cause hormonal imbalances in the body. Generally, a delay of 3–7 days is considered normal.

2. Pathological Causes

1) Thin Endometrium

This condition may occur due to severe damage and thinning of the endometrium following abortion or other uterine surgical procedures, preventing normal endometrial shedding. Hormonal medications such as progesterone capsules or estradiol valerate tablets may be taken under medical guidance for regulation.

2) Polycystic Ovary Syndrome (PCOS)

Excessive secretion of androgens in the body inhibits follicular development and ovulation, resulting in no progesterone production. The endometrium is thus exposed only to estrogen without the balancing effect of progesterone, leading to absent menstruation and failure to conceive. Medications such as progesterone or ethinylestradiol cyproterone acetate tablets may be used under a doctor’s supervision to regulate the menstrual cycle.

In addition, premature ovarian insufficiency could also be a possible cause. It is recommended to visit a gynecologist for ultrasound and sex hormone panel tests (six-item panel), and treatment should be tailored according to the underlying cause to regulate menstruation.
