What are the clinical manifestations of right heart failure?

Introduction
Right heart failure mainly presents with symptoms related to systemic venous congestion. If there is gastrointestinal congestion, symptoms such as abdominal distension and nausea may occur; if there is renal congestion, increased daytime urination and nocturia may be observed; if there is hepatic congestion, symptoms may include liver tenderness and discomfort or fullness in the right upper quadrant. Edema and respiratory symptoms may also develop.

Right heart failure is primarily characterized by symptoms related to systemic circulation congestion. The clinical manifestations can be categorized into gastrointestinal, renal, and hepatic symptoms. Specific analyses are as follows:

1. Gastrointestinal symptoms

Due to gastrointestinal congestion, patients may experience abdominal bloating, loss of appetite, nausea, and vomiting. Gastrointestinal symptoms are the most common in right heart failure.

2. Renal symptoms

When congestion occurs in the kidneys, it affects renal function, leading to increased urine output during the day and nocturia.

3. Hepatic symptoms

Congestion and enlargement of the liver can cause tenderness and a feeling of fullness or discomfort in the right upper abdomen. Persistent chronic right heart failure may also lead to cardiac cirrhosis.

In addition to the above common clinical manifestations, right heart failure may also present with edema and respiratory symptoms. When such symptoms occur, it is recommended to seek medical attention early for diagnosis through physical examination, laboratory tests, imaging studies, and other relevant assessments.
