Video of Percutaneous Tracheostomy
Generally, percutaneous tracheostomy is faster and more effective compared to conventional tracheostomy and is typically used for patients requiring prolonged mechanical ventilation. The specific procedure is as follows:
1. After general anesthesia, position the patient supine with a small pillow under the shoulders and the head extended backward to fully expose the trachea.
2. Select the interspace between the first and second tracheal cartilage rings or between the second and third tracheal cartilage rings as the puncture site. Make a 1–2 cm incision, dissect through the skin and fascia layer by layer, and expose the trachea.
3. Carefully incise the trachea using a scalpel to establish an airway, then insert a tracheostomy tube to ensure airway patency.
4. Secure the tracheostomy tube by tying its attached strap around the neck with a secure knot, and place gauze between the wound and the tracheostomy tube.
5. Confirm correct positioning of the tracheostomy tube and connect it to the ventilator to provide the necessary respiratory support for the patient.
