Birth generally refers to delivery. Delivery with a ROT (right occiput transverse) fetal position is usually difficult, but becomes easier if the fetus rotates into a normal position. The ROT fetal position means that the fetus is not in the typical head-down or breech presentation within the uterus, but rather has its head or buttocks diagonally positioned relative to the pelvis. If a pregnant woman experiences any abnormalities, she should seek medical advice promptly.

1. Difficult Delivery

ROT (Right Occiput Transverse) position means that the fetus inside the uterus is not in the typical cephalic or breech presentation, but rather with the head or buttocks diagonally positioned relative to the pelvis. The ROT position may increase the difficulty and risks of delivery. When the fetal head or buttocks are diagonally aligned with the pelvis, resistance may be encountered during descent, thereby prolonging labor. Because the fetal head or buttocks are not correctly aligned with the pelvic inlet, cervical dilation might be affected and require more time and effort to achieve full dilation. If labor progresses slowly or other complications arise, a cesarean section or assisted delivery techniques such as forceps or vacuum extraction may need to be considered.

2. Easier Delivery

However, some women with mild ROT positions may successfully achieve spontaneous vaginal delivery without additional interventions. Additionally, certain pregnant women may use specific postures and exercises to encourage fetal rotation into a favorable position, thus achieving natural childbirth. For management of ROT position, doctors typically determine the most appropriate mode of delivery based on the individual circumstances of each mother and fetus.
