Hypopituitarism leads to decreased hormone levels in most cases, but this may not occur if hypopituitarism results from abnormalities in pituitary hormone receptors. The pituitary gland is responsible for secreting various hormones. When pituitary function is reduced, the amount of hormones it secretes decreases, thereby affecting the function of downstream target glands and resulting in reduced hormone secretion from these target glands as well.

Pituitary insufficiency usually leads to decreased hormone levels. However, if pituitary insufficiency is caused by abnormal pituitary hormone receptors, it may not directly affect hormone levels in the blood. The pituitary gland is responsible for secreting various hormones. When pituitary function is reduced, the amount of hormones it secretes decreases, thereby affecting the functions of downstream target glands and leading to reduced hormone secretion from these target glands. For example, a decrease in thyroid-stimulating hormone secreted by the pituitary gland can lead to reduced secretion of thyroid hormones by the thyroid gland, which then causes symptoms of hypothyroidism. Similarly, pituitary insufficiency also affects the secretion of adrenal cortical hormones and gonadal hormones, resulting in decreased levels of these hormones.

However, if there are abnormalities in pituitary hormone receptors, pituitary hormones need to bind to receptors on target cells to exert their effects. If the receptors are defective or abnormal, even if the pituitary gland secretes normal amounts of hormones, it cannot effectively stimulate the target glands to secrete hormones. In this case, pituitary function is reduced, but hormone levels in the blood may not be low and might even be elevated.
