How to resolve phimosis with entrapment on one's own

Introduction
In general, after the occurrence of paraphimosis, manual reduction, cold compress to reduce swelling, application of lubricants, gentle traction of the foreskin, and adjustment of body position may be attempted to relieve the condition by oneself. However, these measures are only applicable to mild cases without significant ischemia. Paraphimosis is a medical emergency, and self-management is only suitable for the early stage of onset and cases with mild symptoms.

Generally, after the occurrence of paraphimosis, one may attempt manual reduction, cold compress to reduce swelling, application of lubricants, gentle pulling of the foreskin, and adjusting body position to relieve the condition on their own. However, these methods are only applicable for mild cases without significant ischemia. If abnormalities occur, prompt medical attention is advised. Detailed explanations are as follows:

1. Manual reduction: Place the index and middle fingers of both hands behind the site of foreskin entrapment and the thumbs on the glans penis. Gently push the glans toward the base of the penis while pulling the foreskin forward with the index and middle fingers, slowly reducing the paraphimosis. The movements must be gentle to avoid excessive force that could cause injury.

2. Cold compress to reduce swelling: Wrap an ice pack or cold towel in a clean cloth and apply it to the affected area for 10–15 minutes each time. This helps alleviate local swelling and pain, creating favorable conditions for subsequent manual reduction. Avoid direct contact between ice and skin to prevent frostbite.

3. Application of lubricant: Apply an appropriate amount of medical vaseline or personal lubricant around the foreskin opening and glans edge to reduce friction resistance during reduction, making it easier for the foreskin to slide back into place. Keep hands clean after application to prevent bacterial infection.

4. Gentle foreskin traction: Gently grasp the edge of the foreskin with your fingers and slowly pull outward while using the other hand to gently press and push the glans. Coordinate the pulling and pushing actions to relieve the entrapment. Apply even force without forcibly pulling.

5. Adjusting body position: Try lying on your back and relax the abdominal and perineal muscles to avoid muscle contraction caused by tension, which may worsen the paraphimosis. Maintaining a relaxed physical state facilitates smoother manual reduction.

Paraphimosis is a medical emergency. Self-management is only suitable for the early stage and mild cases. If the above methods fail to achieve reduction within 15–20 minutes, or if symptoms such as localized discoloration (purple appearance), severe pain, or difficulty urinating occur, seek immediate medical attention at the urology department of a hospital. A professional physician should perform reduction—do not delay treatment.
