What to Do When a High Fever Persists
Persistent high fever can be managed through environmental adjustments, physical cooling measures, standardized administration of antipyretic medications, adequate fluid replacement, and close monitoring of disease progression. Prolonged high fever depletes bodily functions; if home management fails to improve the condition, prompt medical attention is essential.
1. Adjust the living environment: Ensure proper indoor ventilation while maintaining a comfortable room temperature. Avoid excessive bundling or covering with thick blankets. Loosen heavy clothing to facilitate heat dissipation—overcovering traps heat and may further elevate body temperature. Create a quiet resting environment to minimize external disturbances, helping the patient conserve energy and reduce unnecessary caloric expenditure.
2. Physical cooling care: Gently wipe major superficial blood vessel areas—including the neck, axillae (armpits), and groin—with lukewarm water to promote heat loss via evaporation. Do not use alcohol for skin wiping, as it poses risks of systemic absorption of harmful substances. If the patient develops chills or shivering, immediately discontinue wiping and ensure adequate warmth. Monitor body temperature regularly to track fluctuations.

3. Standardized use of antipyretic medications: Administer antipyretics only when body temperature reaches the recommended treatment threshold, dosing according to age and weight, and strictly adhering to prescribed intervals between doses. Avoid combining multiple antipyretics indiscriminately to prevent overdose. Note that antipyretics do not produce immediate effects; allow sufficient time for onset of action. Document medication timing and monitor whether temperature declines and subsequently rebounds.
4. Adequate fluid replacement: Sustained high fever leads to significant fluid loss. Drink small amounts of warm water frequently, and consider oral rehydration solutions containing electrolytes. Sufficient hydration supports thermoregulation, reduces the risk of dehydration, and aids in eliminating metabolic waste. Avoid drinking large volumes of water at once or consuming ice-cold beverages, which may irritate the gastrointestinal tract and trigger nausea or vomiting.
5. Close monitoring of clinical changes: Regularly record body temperature and observe mental status, respiratory pattern, facial coloration, and associated symptoms such as vomiting or generalized myalgia. If fever rapidly rebounds shortly after antipyretic administration—or if the patient exhibits marked lethargy—do not delay seeking professional medical evaluation. Maintain detailed records to assist clinicians in rapid assessment of the patient’s condition.
After fever subsides, prioritize bed rest. Choose light, easily digestible foods; maintain appropriate body warmth; avoid exposure to drafts or cold air; and continue monitoring temperature closely to support gradual restoration of normal physiological function.
