Undergoing surgery does not necessarily require mechanical ventilation; ventilators are used only during general anesthesia to assist breathing. For surgeries performed under local or neuraxial (e.g., spinal or epidural) anesthesia, patients maintain spontaneous respiration and do not require ventilator support. Postoperative respiratory abnormalities—such as chest tightness or shortness of breath—warrant prompt medical evaluation and management.

In such cases, a ventilator assumes the role of facilitating gas exchange and ensuring adequate intraoperative oxygenation. Once surgery concludes and anesthetic agents are gradually metabolized and eliminated, spontaneous respiration resumes, allowing for safe ventilator discontinuation (extubation). In contrast, brief procedures performed under local anesthesia—where patients remain fully conscious and unimpaired in respiratory function—do not necessitate ventilator use at any point.

Clinically, the choice of anesthesia is individualized based on both the surgical procedure and the patient’s physiological status. Even for minor operations, patients with compromised cardiopulmonary function may require intraoperative ventilatory support. Postoperatively, respiratory rate and blood oxygen saturation are closely monitored; if signs of respiratory insufficiency emerge, continued ventilatory assistance is provided to maintain stable systemic oxygenation.

Postoperatively, maintaining regular, controlled breathing patterns—supplemented by appropriate deep-breathing exercises—while avoiding tobacco and alcohol exposure and ensuring adequate rest, promotes rapid recovery of cardiopulmonary function to preoperative baseline levels.
